Care Planning & Guidance

Care Assessment & Personalized Plan
Every strong decision starts with a clear picture. Our care assessment is a thoughtful conversation that looks at daily routines, safety, supports, and goals. We listen to the senior and family, consider ADLs/IADLs, identify risks like falls or wandering, and discuss preferences, budget, and timeline. You’ll receive a practical written plan that outlines tailored options such as aging in place with home care, exploring assisted living or memory care, adding home health if ordered by a clinician, or considering hospice when appropriate. The result is confidence, not guesswork.
Aging in Place
If your loved one wishes to remain at home, we design a realistic plan that balances safety, independence, and budget. We map daily routines, recommend sensible home safety upgrades, connect you with community programs and transportation, and identify where home care or technology can make life easier. You’ll receive a simple, sustainable plan complete with contacts and next steps that adapts as needs change.
Hospice Guidance & Family Support
Hospice can be a compassionate layer of support, but it’s often misunderstood. We explain hospice services and coverage in plain language, introduce aligned Flagstaff hospice providers, and prepare families for the first visit and ongoing communication. Throughout care, we remain available for questions, share grief and community resources, and coordinate with the hospice team so your family can focus on presence, comfort, and dignity.
Care Coordination & Transitions
Care Coordination & Navigation
Think of us as your single point of contact for senior care in Flagstaff. We research and introduce reputable providers, organize tours and interviews, align schedules, and help with paperwork so services begin smoothly. We stay in close communication with families and providers to prevent gaps, track what’s working, and adjust when needs change. Our care coordination saves time, reduces stress, and ensures the care plan reflects your loved one’s priorities today, and as things evolve.

Assisted Living & Memory Care Placement Support
When it’s time to explore a community, we turn a complicated search into a guided process. Using the assessment, we narrow the field to assisted living or memory care options that fit your loved one’s needs, personality, and budget. We schedule and prepare you for tours, provide comparison tools, clarify services and staffing, and outline move‑in timelines. We remain by your
side through decision and transition so day one feels prepared, not overwhelming.
Home Care & Home Health Coordination
Staying home can be the right choice with the right support. We match families with reliable Flagstaff home care agencies for companionship, personal care, and respite. When a clinician orders it we can coordinate with home health teams for skilled services. We help set routines, confirm schedules, and establish communication preferences so everyone knows what to expect. After start‑of‑care, we check in and fine‑tune services to keep independence, safety, and quality of life front and center.
Start‑of‑Care Setup & Onboarding
The first days of any new service set the tone. We coordinate schedules and key contacts, organize essential documents, and walk the family through what’s included and who to call for what. After services begin, we check in to confirm everything is on track. This hands‑on onboarding reduces missteps and ensures a confident start for everyone involved.
Ongoing Care Monitoring & Advocacy
Care isn’t a one‑time decision; it’s a journey. We provide regular check‑ins with the senior and family, communicate updates with providers, and resolve issues quickly when something isn’t working. Our monitoring and advocacy keep the plan aligned with current needs, offering peace of mind that a trusted professional is watching details and speaking up on your behalf.
Family & Caregiver Support

Family Communication & Caregiver Coaching
Clear communication lightens the load. We establish a cadence for updates, share practical education on common senior care topics, and coach caregivers on boundaries, burnout prevention, and realistic expectations. The goal is fewer surprises and a more confident care
team.
Dementia & Memory Care Support
Dementia brings unique challenges that deserve patience and skill. We normalize common changes, share gentle communication techniques, and build routines that reduce stress. When a memory care setting in Flagstaff is the right next step, we coordinate options, tours, and transition planning. We also connect families with support groups, respite opportunities, and community resources so no one has to navigate this alone.
Safety & Resources

Home Safety Assessment & Recommendations
A safer home supports dignity and independence. We review entrances, hallways, bathrooms, lighting, and daily pathways to identify fall risks and friction points. Then we recommend straightforward improvements like grab bars, railings, lighting changes, or non‑slip solutions, and connect you with trusted vendors. Our suggestions integrate with your broader aging‑in‑place plan so changes are practical and prioritized.
Community Resources & Referrals
Northern Arizona has a strong network of programs and professionals, if you know where to look. We make targeted introductions to Flagstaff community services, benefits navigation resources, caregiver supports, and trusted professionals such as elder law attorneys, financial planners, and insurance experts. We explain how each piece fits, so your family can tap into the right help at the right time.
