All pricing will be specific to the needs of each family.

Package 1

This starter package centers on a digital care assessment that gently surfaces daily routines, safety considerations, preferences, and goals. After our conversation, you receive a polished PDF summarizing everything we discussed along with specific, local resources including services, support groups, and upcoming events to follow up on or attend. We then coordinate one next step for the single service you choose, such as scheduling an intake, setting a tour, or arranging an introductory meeting. Thirty days later, we check in with you and/or the family to review progress and make light recommendations so your plan keeps moving forward.

Package 2

Package 2 builds on the digital care assessment and PDF resource guide with deeper coordination across every service you select. We handle the outreach and scheduling for multiple intakes, tours, and meetings, then confirm each appointment and send a 24‑hour reminder using your preferred communication method. If transportation is needed, we can arrange it when community resources are available. A 30‑day follow‑up with you and/or your family ensures the chosen services are in motion, questions are answered, and your care plan remains aligned with current needs.

Package 3

Our most comprehensive option pairs the digital care assessment and tailored PDF with full‑scope coordination. We schedule and confirm all meetings, intakes, and tours across your chosen services, provide 24‑hour reminders by text, call, and/or email, and, when resources allow, arrange transportation. When you decide to proceed, we coordinate the start of care for each service so day one is organized and clear. Over the first 30 days, we stay close with periodic check‑ins that look at quality of care, life enrichment, service responsiveness, and anything else affecting short‑term goals. At day 30, you receive an exit report summarizing what’s working, any concerns, and practical recommendations beyond the first month, including refreshed resources, supports, and education where helpful.

Subscription 1

Designed for families who want steady oversight without constant coordination, Subscription 1 includes a monthly check‑in with primary service providers to capture any major changes in condition, diagnosis, or support needs. Each quarter, we reassess the client’s needs and refresh your curated resource list so it stays relevant. We also place one check‑in call per month with the client and/or family to share updates, answer questions, and suggest simple next steps if new needs emerge.

Subscription 2

Subscription 2 increases the cadence and adds proactive navigation. We connect with primary service providers every other week to monitor changes that may require added support. You’ll still receive a quarterly reassessment, plus a monthly check‑in call with the client and/or family. As needs shift, we coordinate meetings with new services, guide you through care changes, declines, transitions, or other aging milestones, and update your resource list in real time so you always know where to turn next.

Subscription 3

For families seeking hands‑on advocacy, Subscription 3 offers bi‑weekly provider check‑ins and a quarterly reassessment, along with a monthly call with the client and/or family. We attend key care meetings or facility plan reviews, coordinate and launch any new services, and continue monitoring to ensure the changes take hold. During major transitions, such as a move to memory care, adding day programming, or starting hospice, we provide close coordination and guidance. Each month, you receive a clear summary report with a plan of action so everyone is aligned on what’s next.
